All disputes must be sent in writing to Customer Service, PO BOX 9217, Des Moines, IA 50306-9217. We must hear from you no later than 60 days after we sent you the first bill on which the error or problem occurred. Please include the following information on the dispute form
1. Your name and account number
2. Dollar amount of suspected error
3. Description of the error and, if possible, an explanation of why you believe the error occurred
